If I am a potential subject for discipline and you intend to question me, I request

union representation.  I invoke the rights listed below.

WEINGARTEN RIGHTS

I HAVE THE FOLLOWING RIGHTS INCIDENT TO AND INTERROGATION

1.  to be informed of the subject matter and to confer privately with my union representative before questioning begins;

2. to have my union representative speak during the interrogation and to ask for clarification;

3. to be advised by my union representative as to how to answer a question; and

4.  to offer information following the questioning.
