Important Year-End Updates: PTO Carry Over and Highmark Wellness Card

This is a friendly reminder that the deadline to submit your Year-End PTO Carry Over Forms is fast approaching. All forms must be completed and submitted by Saturday, December 14, 2024.

The deadline to submit your PTO Carry Over Forms is Saturday, December 14, 2024.

Key Details:

• Hourly Employees:
• Excess PTO hours will be cashed out unless you choose one of the following options:
• Deposit into your 403(b).
• Deposit into your Extended Sick Bank (ESB).
• Deposit all hours (including required carryover) into your ESB.
• Master Bargaining Employees:
• Additional PTO carryover is available through the Additional PTO Carry Over Form.

How to Access Forms:

• Visit KaleidaScope, navigate to Human Resources > HR Forms, and click on Year-End PTO Carryover Forms.
• Forms must be submitted online. Computers are available in HR offices for those without access at work.

Reminder: No changes can be made after the December 14 deadline.

Your Highmark Wellness Debit Card will expire on December 31, 2024.

Use It Before It’s Too Late:

• Accepted at:
• Gym memberships
• Weight Watchers and other health programs
• Medical device retailers
• Specialty grocers (e.g., Braymiller Market)
• Vitamins, supplements, and eye care
• Fitness equipment and retail pharmacies

Important: The card cannot be used for copays, coinsurance, superstores, convenience stores, or warehouse clubs.
